Since fish have their eyes on the sides of their heads, I'd say they can't see in front of them and therefore rely in the the movements of the fish they swim along side of. If said fish don't turn, then our fish don't turn, and hence they end up in the backside of an ostrichman.
His colorectal surgeon will do almost anything for the right amount of cocaine...which isn't that much actually...include mind his own business.
As for how he talks underwater...prerecorded audio clips. Turns out Florence (the pig) is something of a quantum physics savant and calculated precisely what would happen (with in a small range of predictability) on their future swimming adventure, somehow communicated this to Francis, who in turn recorded the audio that you can now read in the cartoon bubbles.
